In the following image, the lake polygon feature class has had its label parameters set to allow polygon labels to be placed outside the polygon if they do not fit inside. You can also specify that the offset will be measured from the feature geometry instead of the edge of the symbology for that feature by using the Measure offset from feature geometry option. The label offset distance can be measured in map units, millimeters, inches, or points. If you could live with fewer labels, this would be easy enough to do occasionally: Turn your grid into a graphic from the first grid properties dialog, right click on the resulting graphic to ungroup the graphic, and then use drawing tools to select, move, and rotate a line of labels. You can set automatic grid properties and also manually modify properties in an open drawing.
